This online form includes: 1) Contact & Emergency & Medical Info, 2) Permission Slip, 3) Photo Release, 4) Parent Meetings Acknowledgement, 5) Family Safety Waiver, 6) Respiratory Viruses Liability Waiver. After your online forms are done, our Program Coordinator will visit you with paper copies of the Charlottesville City Schools’ Consent Form for Sharing Data with us.
