Abundant Life demonstrates God’s love through holistic community development in the Prospect neighborhood. Together, we serve people who live in (or have history in) this community.
This online packet includes: 1) Contact & Emergency & Medical Info, 2) Permission Slip, 3) Photo Release, 4) Family Safety Waiver, 5) Infectious Diseases (Covid) Waiver. After your online forms are done, our Program Coordinator will visit you with paper copies of the Charlottesville City Schools’ Consent Form for Sharing Data with us.
That’s a lot! But it helps us care well for your child(ren)!
